OCTOBER 18 & 19 – ESCAPE TO THE
RAINFOREST If you are participating in all three workshops you will receive 15% discount. Please make cheques payable to The Cotton Tree Quilt Shop. A deposit of $100 is required to reserve your space. A Visa or MasterCard number or cheque is required upon registration for all classes. GST will be added to workshop fees. The balance of the workshop fees is due by August 23, 2008. If you are paying by cheque please include a postdated cheque for the balance due on August 23, 2008. If paying by credit card the remaining amount will be processed on August 23, 2008. If you must cancel, refunds will not be given. You may transfer your Registration to another person of your choice. Participants accept full responsibility for their wellness & personal safety and for that of their belongings while in attendance at the workshops and agree to indemnify and hold harmless The Cotton Tree Quilt Shop, its principals, possessions, and representatives for any losses or injuries incurred. Participant agrees by signing below to accept all terms.
